The Emergency Medical Fund (EMF) provides financial aid to non-profit animal welfare groups needing assistance with special medical costs. PACC911 EMERGENCY MEDICAL FUND (EMF) GUIDELINES
EFFECTIVE 1/10/08

1.   Only active PACC911 Partners will be eligible for funding through the EMF Program, 
      AS LONG AS THE FUND IS SOLVENT.
An active partner is one that participates in 
      at least two PACC911 adoption events per year.


2.   The PACC911 EMF Committee shall maintain an unbiased stance as it relates to if 
      and how monies from the funds are dispensed.

3.   The EMF shall be primarily capitalized by means of specially designated fundraising 
      events.

4.   Fund requests must be for animals that are in the care of a PACC911 Partner at 
      time of required treatment.
Treatment for previously adopted animals will not be 
      considered.
Application for funding may be submitted up to 60 days after a 
      procedure has been performed.

5.   Treatment must be of an emergency or life-saving nature, or relieve a chronic or 
      debilitating condition resulting in a greater possibility for adoption.
Diagnostic 
      procedures & associated medication relating to these situations will also be funded. 
      However, follow up medical treatment, spay/neuter and routine vet care are not 
      eligible.
Age, overall health, chances of survival, and adoptability of an animal will be
      taken into account when reviewing applications for funding.
No bill is too small to 
       submit!


6.   To expedite the funding process, all PACC911 Partners desiring funding under the 
      EMF program should register with PACC911 in advance using the official EMF 
      
Registration Form accompanied by their official 501(c)3 form, or evidence of 501C3 
      Application.

7.   An official
EMF Request for Medical Assistance Form must be completed, reviewed, 
      and approved for each treatment before monies can be dispensed. This form can be 

       mailed or faxed
, by the requesting PACC911 Partner. The form must be signed by a 
      Director of the Partner organization and be accompanied by an estimate or actual 
      invoice from the performing veterinarian.

8.   Requests for funds will be reviewed, and eligibility determined by a majority vote of 
      the EMF Committee members, within one week of receipt. In the event of a tie vote, 
      the PACC911 President will make the ultimate decision as to if, and how much 
      funding, will be dispensed.

9.   Checks for EMF monies will be made payable to the PACC911 Partner and will be 
      mailed to the designated recipient within 10 days of approval.

10.  If requests for EMF monies are made prior to the actual treatment, proof of 
      treatment must be provided by the Partner organization within two weeks of 
      treatment. Acceptable proof of treatment shall be a paid invoice, a letter from the 
      veterinarian performing treatment, or actual treatment record. Failure to provide
      such proof will result in that Partner’s forfeiture of future EMF funds.
 
11.  Reasonable and customary charges will be used as a benchmark for calculations and 
      payment of EMF monies.

12.  Amount granted will not exceed 50% of required veterinary treatment, not to 
      exceed $1,600.00 per calendar year. 
              examples:  $187.50 will be granted for a treatment costing $250 (=75%)
                             $375 granted for a treatment costing $500 (=75%) 
                             $750 granted for a treatment costing $1,000 (=75%) 
                             $1,600 for a treatment costing $2,133 or more

13.  The $1,600.00 MAX funding can be used on one animal or several. Groups who have 
      received funding less than the $1,600.00 can apply for additional funding up to the 
      Maximum amount.

14.  Separate EMF files will be maintained for each Partner organization. All requests for 
      EMF monies will be logged by a designated EMF Committee member, whether funds 
      are approved or not. An archive shall be kept for reference, publicity, etc

15.  At the end of each calendar year, PACC911 will actively solicit feedback from all 
      Partner organizations, who have applied for funding. Results shall be used to 
      evaluate and revise the EMF Program on an annual basis, as deemed necessary.
